The Stefan Banach Medal is awarded to individuals by the Presidium of the Polish Academy of Sciences (PAN) in recognition of outstanding contributions to the development of mathematical sciences. The Stefan Banach Medal was established in 1992, marking the centenary of Stefan Banach's birth.

Annex No. 1 to Resolution No. 11/2021. Presidium of the Polish Academy of Sciences of March 16, 2021.
§ 1
The Stefan Banach Medal is awarded by resolution of the Presidium of the Polish Academy of Sciences to a person recommended by the Chapter of the Stefan Banach Medal.
§2

The Stefan Banach Medal Chapter, hereinafter referred to as the "Chapter," is appointed for the term of the Presidium of the Polish Academy of Sciences.

On behalf of the Presidium of the Polish Academy of Sciences, the Mathematics Committee of the Polish Academy of Sciences appoints a five-member Chapter for the term, composed of individuals previously awarded the Medal or members of the Polish Academy of Sciences.

Applications for the Medal may be submitted to the Chapter by:
1) Members of the Polish Academy of Sciences in a number not less than three;
2) The senate of a university, scientific councils of institutes of the Polish Academy of Sciences, and other research units authorized to award the degree of habilitated doctor in the field of mathematical sciences;
3) Members of the Chapter in a number not less than three - hereinafter referred to as "Applicants."

Applicants are obligated to maintain confidentiality regarding the nomination, particularly not informing the candidate of the initiative.

Applicants recommending a candidate for the Medal should maintain impartiality and avoid conflicts of interest. Applicants may not nominate a candidate with whom they have:
1) Direct employment relationship;
2) Marital relationship, kinship up to the second degree, affinity up to the first degree, adoption, guardianship, or curatorship;
3) Any other relationship that may affect the objective nature of the nomination.

An application for the Medal may be withdrawn at the request of the Applicant.

Applications referred to in paragraph 1 are considered once a calendar year. Information on the call for Medal candidates, along with the deadline for submitting applications, will be posted on the website of the Mathematics Committee of the Polish Academy of Sciences.

An application for the Medal should include:
- The candidate's name, academic degree and title, residential address, and place of employment;

A justification containing a description of the candidate's scientific achievements and their contributions to the advancement of Polish science.

The Chapter deliberates in meetings.

The Chapter's meetings may be conducted using electronic communication means, ensuring in particular:
1) Real-time transmission of the session between its participants,
2) Multilateral real-time communication during which participants can speak,
- with the necessary safety measures in place.
§ 6

First, the Chapter evaluates the formal compliance of the application.

If the requirements specified in § 4 are not met, the Chapter requests the Applicants to supplement the application within a specified period.
§ 7

The Chapter selects a candidate by resolution, adopted in a secret vote by a simple majority of votes in the presence of at least three Chapter members.

After adopting the resolution, the Chapter asks the candidate to agree to be honored with the Medal.

The Chapter provides the Presidium of the Polish Academy of Sciences with comprehensive substantive information about the candidate's scientific achievements for the Stefan Banach Medal.

The Chapter reserves the right not to select any candidate for the Medal from the submitted applications.
§8
The Chapter's resolution is presented by the Chapter's Chairman at the meeting of the Presidium of the Polish Academy of Sciences.
§ 9

The Medal is presented to the honoree by the President of the Polish Academy of Sciences or one of the Vice Presidents of the Polish Academy of Sciences.

The honoree is asked to deliver the "Banach Lecture" at the International Mathematical Center in Warsaw.
-
The Chapter organizes the ceremony for presenting the Medal and delivering the "Banach Lecture" mentioned in paragraph 2.

on the regulations for awarding the Stefan Banach Medal
Based on § 27 para. 1 and para. 4 of the Statute of the Polish Academy of Sciences, constituting an annex to Resolution No. 8/2010 of the General Assembly of the Polish Academy of Sciences of November 24, 2010, on the Statute of the Polish Academy of Sciences, approved by the Prime Minister on December 28, 2010, amended by Resolution No. 8/2012 of the General Assembly of the Polish Academy of Sciences of September 25, 2012, approved by the Prime Minister on November 26, 2012, amended by Resolution No. 1/2015 of the General Assembly of the Polish Academy of Sciences of March 19, 2015, approved by the Prime Minister on June 8, 2015, amended by Resolution No. 4/2018 of the General Assembly of the Polish Academy of Sciences of June 21, 2018, approved by the Prime Minister on September 6, 2018, and amended by Resolution No. 4/2020 of the General Assembly of the Polish Academy of Sciences of December 3, 2020, approved by the Prime Minister on February 8, 2021, the Presidium of the Polish Academy of Sciences hereby resolves as follows:
§ 1
- The Presidium of the Polish Academy of Sciences establishes the Regulations of the Stefan Banach Medal, hereinafter referred to as the "Regulations," which constitute Annex No. 1 to this resolution.
- The Regulations specify the procedures for the work and selection of members of the Stefan Banach Medal Chapter and the principles for awarding it.
- The Presidium of the Polish Academy of Sciences may award the Stefan Banach Medal, hereinafter referred to as the "Medal," to individual persons in recognition of outstanding contributions to the development of mathematical sciences.
- The Presidium of the Polish Academy of Sciences awards one Medal per calendar year, by resolution adopted by a simple majority of votes in the presence of at least half of its members.
- The Medal is honorary and can only be received once.
§2
- The Medal is in the shape of a circle with a diameter of 90 mm and is made of gilded metal. On the obverse of the Medal is a portrait of Stefan Banach, and on the reverse, there is an inscription in the outer rim: "To the Genius Mathematician, the Polish Academy of Sciences," in the center: "Stefan Banach 1892-1945 Creator of Functional Analysis," and below it, the quote: "My greatest discovery is Stefan Banach" Hugo Steinhaus.
- The design of the Medal is Annex No. 2 to this resolution.
§ 3
- The person honored with the Medal receives a diploma in Polish, signed by the President of the Polish Academy of Sciences and bearing the round seal of the Polish Academy of Sciences.
- The diploma design is Annex No. 3 to this resolution.
§4
The Stefan Banach Medal awarded under previous regulations remains in effect.
§5
Resolution No. 5/97 of the Presidium of PAN of February 3, 1997, on the regulations for awarding the Stefan Banach Medal of the Polish Academy of Sciences, and Resolution No. 4/2021 of the Presidium of the Polish Academy of Sciences of January 19, 2021, amending the resolution on the regulations for awarding the Stefan Banach Medal of the Polish Academy of Sciences, are hereby repealed.

The resolution shall enter into force on the date of adoption.
JUSTIFICATION
To honor outstanding contributions to the development of mathematical sciences, the Presidium of the Polish Academy of Sciences awards the Stefan Banach Medal to individual persons. This is a highly prestigious honor.
The Regulations of the Medal require necessary changes to align its content with the current Statute of the Polish Academy of Sciences.
